The form may then be emailed back to clinician prior to the appointment for review and research of drug nutrient interactions to be reviewed during the counseling session.

The food intake history form is designed for use in private nutrition practices. The form can be emailed to a client in advance of their nutrition appointment. The client may open the form in Adobe Acrobat 8.x or newer, enter their food intake history for a given day, save the file, and email it back to the dietitian for evaluation. The form, with the data entered by the client, may be printed and included in the hard-copy chart or saved as an electronic record.

The seven day activity log is designed to report a client's activity for a 7 day period. It is broken down into 6 fields.

  • Day / Date: The day of the week being reported.
  • Activity: The Activity being performed by the client.
  • Duration: The amount of time spend performing an activity in minutes.
  • Distance: The distance covered while performing an activity. Can be used with duration to calculate speed.
  • Intensity: Based on the 5 heart rate zones, it is a sliding 1 - 5 scale.
  • Average Heart Rate: When available, the user should report their average heart rate while performing the activity

This information is used to calculate both a daily and weekly average Kcalorie expenditure for a client. It is most effectively calculated when using a kcal/lb/min or Kcal/kg/min equation that utilizes the client's actual body weight to calculate Kcalories being expended.
